چکیده

CADAVERIC kidney graft survival is influenced by many different factors. Two such factors are matching for the HLA-DR determinants between donor and recipient and blood transfusion(s) given to the recipient before transplantation. Indeed, recent reports have shown that matching for the HLA-DR determinants and the effect of pre-transplant blood transfusions play an important role in predicting kidney graft outcome in human cadaveric renal transplantation 1, 2, 3, 4, 5. Most of these retrospective studies were critisized because of selection criteria, insufficient accuracy, heterogeneity, patient management etc. To tackle the problem of retrospective studies as many patients as possible on the Eurotransplant waiting list were typed prospectively for the HLA-DR determinants. Kidney donors were typed before or at time of transplantation and are therefore considered as prospective.

Superiority of B locus matching over other HLA matching in renal graft survival.

Graft survival after 348 consecutive first cadaver-donor renal transplants was significantly improved by HLA matching when recipients who had received pretransplant blood transfusions were matched with their kidney donor for two HLA-B locus antigens. No other type of HLA matching significantly improved graft survival in transfused recipients nor did any type of HLA matching in non-transfused re...

HLA matching and cadaver kidney transplantation — status 1984

The effect of HLA matching on cadaver kidney graft survival was analysed in over 9000 transplants. Matching for HLA-A and -B accounted for an improvement of 8% in the one-year survival rate, matching for HLA-DR for 10%, and matching for HLA-B + DR for 19%. The matching effect of the HLA-B and HLA-DR loci was additive. Patients without pre-transplant transfusions had lower graft survival rates t...

HLA-DRw6 and renal allograft rejection.

HLA-DRw6-positive patients are "high responders" to certain renal allograft antigens. A study was therefore conducted of the outcome of 247 first renal allografts in 74 DRw6-positive and 173 DRw6-negative recipients. The effectiveness of matching for HLA-DR determinants in both groups was also analysed. The one-year graft survival in DRw6-positive patients was 59% as compared with 75% in DRw6-n...

Should the allocation of cadaveric kidneys for transplantation be based on HLA matching?

In 1996 the European centres participating in the Eurotransplant organization agreed to the implementation of a new allocation system for kidneys of postmortal donors (Eurotransplant Kidney Allocation System, ETKAS). The proposed procedure was based on the following five criteria: HLA mismatch grade, waiting time, mismatch probability (a correction factor increasing the chances for patients wit...

HLA-D, DR—CLINICAL SIGNIFICANCE Α New Strategy to Improve Kidney Graft Survival: The Induction of CML Nonresponsiveness

Α LTHOUGH immunosuppression made *"*· renal allografting possible, the excellent results in ABO-compatible HLA-identical siblings showed that optimal matching allowed the immunosuppressive dosage to be significantly reduced. Attempts to extrapolate the sibling findings to unrelated donorrecipient pairs have until recently met with only partial success, however. During the last half year evidenc...
